24/7 Crisis Support

Screening, evaluation, stabilization, and urgent behavioral health support when it is needed.

Community-Based Care

Serving Palm Beach County

Care and support for individuals, families, and community partners across Palm Beach County.

Continuum Services

Ongoing care that supports treatment, stability, and connection to services.

SCMHC provides outpatient and community-based services for people who need continued treatment, medication support, case management, care coordination, or help staying connected to the right level of behavioral health care.

Outpatient Therapy

Outpatient therapy supports emotional, behavioral, and mental health needs through individualized treatment. Services may work alongside psychiatric care, medication support, case management, and other supports as part of a broader treatment plan.

Case Management

Case management helps individuals identify needs, barriers, and strengths so an individualized service plan can be developed. Services may include linkage to care, advocacy, follow-up, and support with maintaining stability in the community.

Substance Use Support

Substance use support may be included as part of SCMHC’s broader behavioral health continuum, especially when mental health and substance use needs overlap.

SOAR

SOAR provides assistance to eligible individuals who are navigating the process of applying for Social Security disability benefits when behavioral health conditions make the process difficult to manage independently.

Psychiatric Evaluation & Medication Services

SCMHC provides psychiatric evaluation and medication-related treatment for outpatient clients. Adults served by SCMHC may also have access to the medication clinic and pharmacy services as part of their care.

Care Coordination

Care Coordination supports individuals with complex or high-level behavioral health needs by helping connect them with treatment, housing, insurance assistance, medication management, therapy services, peer supports, vocational resources, and other community supports.

Forensic / Jail Diversion

SCMHC’s Forensic Mental Health program supports adults with serious mental health conditions who are involved with the criminal justice system. Services may include diversion to community-based treatment when appropriate, coordination with courts and treatment providers, competency-related support, discharge planning, and intensive community-based services.

Clients are referred through Circuit 15 Division T or other Florida mental health court systems, so this is not a general walk-in service.

Residential and Recovery Support

Structured care and practical support for greater stability and independence.

SCMHC offers residential and recovery-focused services that help adults build stability, strengthen life skills, and connect with the supports needed for greater independence in the community.
